Currently, Fidelity Charitable provides IRS Form 8283 as a courtesy to our donors when complete tax recipient information is received for a contribution but does not accept consolidation requests. If you would like to consolidate your deductions, consult with your tax advisor. If needed, a blank IRS Form 8283 is available on the IRS website.
