Complete the attached Letter of Instruction to contribute mutual funds and securities held at firms other than Fidelity. This letter will be pre-filled if you use the online contribution option when you Set up a Giving Account online >.
Letter of Authorization (PDF) for Fidelity Brokerage Customers Complete the one-time Letter of Authorization to make contributions from your non-retirement Fidelity brokerage account to Fidelity Charitable. You can submit it online when you make a contribution, or you can print the attached form, complete and sign it, then fax or mail it.

Irrevocable Stock or Bond Power Form (PDF) Required for contributions of physical stock or bond certificates. This form must be completed, signed and attached to the certificate.

|Giving Account access||
Giving Account Access Form (PDF) This form allows you to authorize an individual, such as your financial advisor or another interested party, to obtain information or perform transactions on your behalf.
Firm Access Form (PDF) This form allows an advisory firm to enable a team of their associates to access a Giving Account, in accordance with the Advisor Authorization level selected by the Account Holder.
Your advisor's firm can also register online for firm-wide access.

Giving Account Change Form (PDF) This form allows you to make changes to the personal information associated with the Giving Account, add additional donors, and add or change successor elections. If you prefer, you may also log in to change your address or successor information online >
